The discount is € 50 excl. VAT (€ 60,50 incl.) per order, once, regardless of the brand of your old charger — EVBox, Alfen, Zaptec, Easee, Wallbox or anything else. You do not have to send anything in to get it. Trade-in and the € 50 referral discount do not stack: one discount per order.
If the hardware is healthy and all you lost is the platform — registration, sessions, cost allocation — replacing it is a waste.

An honest caveat: whether your unit can be pointed at a different OCPP backend depends on the model and how it was installed — on some models that is only possible with a service tool or through an installer.
